Scholarship Request

I offer scholarships to women who can benefit from my services but are financially challenged during this season of their lives. Special consideration will be given to scholarship awards when the recommendation comes from someone working in a non-profit or ministry that serves women and those they love. This is designated below as Agent and Agency.

Both the recommending agent and the person recommended for a scholarship must fill out and sign their portion of this form. Scholarships are provided on either a discounted or free basis. I look forward to partnering with you! We gain so much when we work together.
